Alcohol is woven into how many of us socialise, unwind and mark the end of a long day. In today’s busy world, a glass of wine can feel like one of the few moments of calm in a relentless schedule.
This article isn’t about telling you to avoid alcohol altogether. But instead building an understanding of what’s actually happening hormonally when we drink. This knowledge can empower us to make more informed choices about what works for each of us, particularly during perimenopause – a time when alcohol may have a bigger impact.
Why alcohol impacts us differently in midlife
Before we get to hormones, it’s worth noting the way alcohol may be processed differently (and why hangovers feel so much worse!) as we enter midlife. As we age, changes in body composition, hydration, sleep quality, liver function and overall sensitivity to alcohol can all affect how we respond to a drink. This means the same two glasses of pinot grigio that barely registered in your twenties could have a noticeably bigger impact in your forties and fifties.
While many women recognise this shift instinctively, the link that’s less often made is the possible impact on hormones and perimenopause symptoms. Feeling rough after a few drinks is often thought of simply as the price we pay. But if we dig just a little deeper, it’s worth knowing a little more about why our body imposes this ‘midlife drinking tax’ so you can explore whether a few simple tweaks could create a smoother ride through perimenopause.
The under-recognised link between alcohol and hormones
A common misconception about perimenopause is that it is a period of steady oestrogen decline. In reality, many women will often experience a drop in progesterone first, with oestrogen levels swinging up and down before declining during late perimenopause and post-menopause. Therefore, often symptoms during the earlier stages of perimenopause can be experienced due to oestrogen surges and low progesterone, rather than declining oestrogen.
This is where the link with alcohol comes in. When we drink, the liver prioritises processing the alcohol. But the liver is also responsible for clearing oestrogen, so alcohol can change the way oestrogen and other sex hormones are metabolised.
How efficiently your body clears oestrogen can depend partly on your genetics, your nutrient status, your gut, and how much you’re asking your liver to process. It can be very helpful to understand more about this, which is why we use functional tests such as the DUTCH test to understand not just hormone levels, but how well you are able to metabolise and detoxify them.
Alcohol and sleep
Alcohol can also disrupt sleep, especially in the second half of the night. While its sedative effects may help you drop off initially, it can lead to lighter sleep as its metabolised, with possible impacts including a faster heart rate, dehydration, night sweats, or feeling more alert in the early hours. Since sleep plays a key role in hormonal regulation, there may be an additional hormonal impact in addition to feeling less rested.
The role of histamine
Histamine is best known for allergies, insect bites and hay fever season. But it also has a two-way relationship with oestrogen that rarely gets mentioned. This is especially relevant in perimenopause when hormones are changing. Not only does histamine come with its own symptoms (flushing, headaches, itchy skin, racing heart, digestive issues), it may also amplify perimenopause symptoms, something that alcohol can feed into.
Higher oestrogen not only triggers the release of more histamine, but it can also lower the enzyme (DAO) that breaks it down. Histamine can also stimulate more oestrogen, creating its own feedback loop.
Progesterone on the other hand, does the opposite, inhibiting histamine release and boosting the DAO enzyme that helps histamine to get cleared.
Now let’s put that in the context of perimenopause. Progesterone is often the first hormone to decline, whilst oestrogen can fluctuate and may swing high during early perimenopause. This is like lose the brake for histamine whilst the accelerator is being pressed harder.
Alcohol can feed into this further. Certain alcoholic drinks like red wine and beer contain histamine themselves, and alcohol can also inhibit the enzyme that breaks down histamine. What’s more, alcohol also stimulates the body to release more histamine!
This link between hormones, histamine and alcohol is a key reason to rethink your drinking patterns if you’re struggling with perimenopause symptoms.
What this might look like day to day
For some women, alcohol may contribute to:
- Worsened mood swings and anxiety
- Increased fatigue and brain fog
- More noticeable PMS or perimenopausal symptoms
- Headaches or flushing after certain drinks
- Disrupted sleep, particularly waking in the early hours
While we can’t tie any of these symptoms directly to alcohol, it can be helpful to get curious about whether alcohol might be driving or worsening any hormonal symptoms you may experience.
Navigating the perimenopause alcohol tax
During perimenopause, alcohol can add further strain to a hormonal picture that’s already off-balance.
The real kicker, though, is that many of us drink to manage the very symptoms that alcohol can worsen. The glass of wine to soothe anxiety or night cap to help you sleep after a long day. It’s a well-intentioned and understandable coping mechanism.
And if it sounds like you, you’re not alone: a 2023 Newson Clinic survey of almost 1,200 women found that one in three reported drinking more since becoming perimenopausal or menopausal.
However, when we understand the hormonal mechanisms at play, we start to see why the relief is temporary, and how the pattern can sometimes create a vicious cycle.

Rather than focusing on giving up alcohol altogether, think of yourself as a detective and start noticing how your body responds. It helps to note how you feel before you begin, so you’ve got a baseline to compare against.
Alongside reducing your intake for a few weeks to see what changes, pay attention to patterns like these:
Does your tolerance change across your cycle? You might feel absolutely fine after two glasses one week, then find the same amount leaves you anxious, exhausted or nursing a headache a fortnight later.
If you’re still having periods, notice the days before your period. Many women tell us that avoiding alcohol in this window helps reduce hormonal headaches, steadies energy and leaves them feeling much more like themselves.
Watch how you respond around ovulation, when oestrogen is often at its highest. This can matter especially if flushing, headaches or histamine-type symptoms are part of your picture.
Notice which drinks affect you most. Red wine is a common trigger thanks to its histamine content, while some women find spirits or un-aged white wines like Sauvignon Blanc or Pinot Grigio may have fewer consequences.
Pay attention to your sleep. Waking between 2am and 4am, a higher overnight resting heart rate (which can be tracked on most wearables), or waking already exhausted rather than rested, can all be clues.
Track your mood over the following day or two. Does alcohol leave you more anxious, irritable or flat than you expected?
Keep an eye on digestion too. Bloating, reflux or changes in bowel habits can become more noticeable after drinking.
From there, a few practical tweaks can make a real difference:
Experiment with timing, not just quantity. Many women find they can still enjoy a drink socially but feel dramatically better by steering clear during certain parts of their cycle, or at high-stress times.
Support your body before and after. A protein-rich meal beforehand, plenty of water and not drinking on an empty stomach won’t undo alcohol’s hormonal effects, but they can help to ease some of the physical strain.
Notice when you’re reaching for a glass. Is it to celebrate, or because you’re exhausted, wired or trying to switch off? Recognising the reason is often the first step to meeting that need another way.
One thing we’ve noticed in clinic over the years is that women rarely regret taking a break from alcohol. Far more often, they’re surprised by how different they feel: improved sleep, a calmer mood, more energy and a bigger than expected improvement in hormonal symptoms. You don’t have to give it up for good to feel the benefit of understanding it better. The key is knowing when alcohol serves you and when it doesn’t, so your choices become conscious ones rather than habit.
