Women’s Health Assessment

We’re excited to dive into all that’s going on with your health and hormones, provide you with insight, and most importantly, next steps.

Look out for an onboarding email that should be waiting for you in your inbox (if you don’t see it check junk). This email will also layout all the information below.

GETTING SET UP

1. Complete the following forms:

  • Health Questionnaire – {click to complete}
  • Food, Mood and Symptom Diary {click to complete} (Please complete this on your phone/laptop first and then enter the 4 days all together.)

2. Blood Tests:

  • Once you’ve completed your Health Questionnaire, you’ll be invited to join our secure client platform, Practice Better.
  • If you’ve had any blood tests taken in the last 6 months (via your GP or privately), you’ll be invited to upload them. This isn’t required, but it’s a helpful addition. [ Click here for full instructions on uploading your blood test documents]
  • If you don’t have recent blood test results but would like to order them privately for inclusion in your Women’s Health Assessment, your Well Nourished Club practitioner will be in touch to organise this with you.

Once you’ve submitted your Health QuestionnaireSymptom Diary, and blood test results (if applicable), you’ll be invited to book your 1:1 consultation.

Please do not book your 1:1 session until invited by one of our practitioners. This ensures we have everything we need to make the most of our time together.
